Cantata Health Solutions pharmacy software is designed to help your pharmacy clinicians facilitate accurate, medication decisions, for medication management such as eMar & CPOE.
Medication & Inventory Management
Uniquely designed as a solution to the difficulties of manual order processing, Convergence Care Rx provides pharmacists with a clinical decision support (CDS) system that combines the elements of a conventional pharmacy system — order and inventory management — with an added focus on medication safety and user preference.
While our base system is used primarily in inpatient pharmacy practice, our software is adequately equipped to handle pharmacy transactions on an outpatient basis. Our integrated ambulatory pharmacy management software supports all order types (IV, Oral/IM, LVP, SVP, TPN, Topicals, etc) and both unit dose and non-unit dose drug distribution. We utilize a prescription model of order entry, label, adjudication, and refill via assigned prescription numbers, and generate printable prescriptions electronically to eliminate problems of illegibility. Cantata also supports ambulatory drug administration with our eMAR, point-of-care drug administration safety software.
Clinical Decision Support
Clinical Decision Support (CDS) systems have a number of important benefits, including an improvement in the quality of care, patient outcomes, and staff efficiency, as well as a reduction of errors, adverse drug events, and superfluous spending. Our products not only enable clinicians with intuitive decision support software but do so intelligently so as to avoid encumbering clinicians with alert fatigue.
Clinical Intervention Module
The Clinical Intervention Module, found within our pharmacy management system, enables pharmacists to review medication orders, monitor for adverse drug events, oversee drug utilization, and provide drug information and patient medication counseling.
Clinical Interventions can be documented at the patient level (attached to a specific patient and not to any particular order) or at the order level (attached to a specific patient order).
Access and Design Electronic Clinical Documentation
A powerful and cutting-edge EHR system, to be fully capable of assisting clinicians, must be equipped with a dynamic and flexible solution to facilitate patient data collection. The evidence has been made clear in recent years: paper forms fatigue clinical staff, burdens clinicians with inefficiency, and can leave clinicians vulnerable to error. Healthcare providers are now turning to electronic patient records to minimize production costs, do away with printing and file management, increase patient privacy and security, and make form access easier, saving clinicians precious time and improving patient care.
Audit Logs & Clinical Reports
Cantata utilizes audit logs & clinical reports to accurately record and relay important patient care information. Integrated within our CPOE, eMAR, and pharmacy management applications, all logs and reports can be shared easily and securely between departments, improving workflow and providing an added level of security during patient care. These two essential components of our EHR track and document all transactions, changes, and inquiries to provide the administration with a detailed report of all end-user activity. The audit history offers a comprehensive understanding, along with the complete patient profile, of a patient’s unique medical history.
Discharge Medication Instruction Sheet
The Discharge Medication Instruction Sheet is a printed list of all medications that the patient needs to take after discharge. The sheet includes information such as the drug name, how and when to take the medication, and the purpose of the medication. It is given to the patient upon release from the facility and includes the patient’s name, allergies, and admitting physician printed near the bottom of the form for follow-up visits.
Integrated EHR Diagnostic Results
Relaying information between departments is one of the most important functions of an EHR. Cantata incorporates diagnostic results into each step of the care process, allowing physicians, pharmacists, and nurses to view, record, and assess the results to better treat their patients. Our integrated EHR diagnostic results improve workflow and reduce errors by providing ensuring clinicians have access to critical patient information.
Complete Patient Profiles
Critical patient information is made readily accessible to clinicians of all disciplines with a complete patient profile. Each profile includes personal information, patient photo, demographics, allergies, vitals, diagnoses, patient history, integrated lab and radiology results, admission date, medication lists, and much more. Supplying clinicians with a secure and convenient method of managing and communicating patient information is one of the many ways in which Cantata’s clinical decision support facilitates responsible patient care and improves overall outcomes.
Comprehensive eForms Library
Our comprehensive eForm library helps to simplify the transition into electronic documentation by providing access to an extensive catalog of pre-configured electronic forms. Our database contains over 600 existing templates that are currently in live use at our clients’ facilities. These eForms conform to the needs of numerous specialty disciplines and hospital departments upon deployment.
Fully Integrated Database with Web-Based Design
One unified database, one integrated EHR for all. Our complete EHR functions from this fundamental principle. All information is shared across our individual clinical applications with a single database to facilitate interdepartmental communication. Our web-based design allows clinicians to access patient charts, test results, and all other necessary information from a remote location, with no information stored on site.
Generic Alternative Decision Module
Pharmacists have access to our Generic Alternative Decision Module, a useful tool within Convergence Rx that suggests cost-beneficial equivalency substitutions for items utilized during order fulfillment. Equivalencies are medication items, or combinations of items, that are used as a substitute for an ordered item. Examples include using multiple doses of one medication to equal a higher strength of the same medication; using a generic equivalent medication as a substitute; and using a therapeutic equivalent medication to substitute for a medication of the same therapeutic category.
