The solution supports secure messaging with providers. It allows the consumer to complete forms before a visit and to complete forms after a visit, including satisfaction surveys. It provides details on past visits and future appointments and supports alerts and notifications.
Convergence offers a directory of available programs and services allowing for care collaboration across both organizations and sectors. With its closed-loop referral workflow, it further supports configurable alerting to ensure people get to their appointments and tracks referral outcomes. Convergence Referral Management focuses on providing the data infrastructure to support the coordination of services. Referral decisions will be based on real-time access to health and administrative records. As made available, information may include program interactions, health summaries, assessments, PMP records, and social determinants of health data.
Vaccination tracking supports scheduling, tracking, and reporting. The Convergence Care Platform solution supports integration with immunization administration and two-way communication with immunization registries.
The Convergence Provider Portal boosts care team productivity with a secure and simple way to share clinical alerts, lab results, and other essential messages to improve staff workflow.
